Dog Intake Form

TXCR is not able to take in aggressive or unsocialized animals with behavioral issues at this time. We do not intake any dogs over 12 pounds. Please fill out all of the required fields for the intake form and a volunteer will get back to you as soon as possible. Our intake process can take 2-4 weeks. If you have more than one dog, you will need to fill out an individual intake form per dog. Thank you.
